Everyone bitches and moans about the bike part in battletoads, anyone who does that never got to experience this nightmare:
At least the bike part had checkpoints and you could see what was going to be the obstacle (for most of the stage). This one requires insane finger activity.
Now, I'm only going to mention level/stages/courses/missions that I've played and beaten from any game. Keep in mind that I've mostly played old games, so you'll mainly see those here. Also, these stages will be ones I still struggle with today.

Yea, for those who've played GTA:SA, you know what I'm talking about. Ya know, for a mission in the first chapter, it isn't all that easy!
Yoshi's Island: Kamek's Revenge (getting 100 points)
This is, without a doubt, the most difficult thing there is to do in Yoshi's island. With the helicopter AND skiing section coming in AFTER the checkpoints, you'll have to do both runs perfectly and remember where all the flowers and red coins are, also recognize the layout. you'll definitely want to gain a few lives before playing this stage if you end up missing that 1 red point or flower.
Yugioh: Reshef Of Destruction - Reshef The Dark Bring
For those who played yugioh, or use to play, have you ever played this game? And if you have... did you beat it? Because, this game is a nightmare. The amount of grinding it takes to actually make progress is astounding in this game that you'll almost always never grind enough. Not to mention that the NPC's are cheap AF (Some cards you can only have 1 of in your deck. They can have up to 3 copies of those cards.) But when we get to reshef, it's just one hell of a nightmare. First off, you have to duel Pegasus first, who has 20000 life points... seriously? You only have 8000! Also he has relinquished, Thousand eyes restrict, and change of heart... 3 copies of each too! They all can take control of one of your monsters which, and trust me, is the WORST THING THAT CAN HAPPEN! But when you do beat him, you duel Reshef, who has 40000 life points...WHAT?! Not only that,but is deck in just unfair. He even has all 3 Egyptian Gods! If you do ever play this game and make it here... good luck. Although the whole game itself is very difficult. Oh and one more thing, the life points you have leftover after dueling Pegasus carry over to your duel with Reshef...
Reshef of Destruction is ridiculously difficult at the end. I have no idea what the developers of the game were thinking when they gave Pegasus and Reshef an absurd amount of life points, not to mention their cards are insanely overpowered.
This level always makes you eat your skin when you die. Especially when you approach boss, in the place where the water begins to rise and if your timing is bad, you'll be stuck here forever unless you crouch. It will descend the water and it's the only good thing in the game that saves your life from murders or self-suicides.

2. SUPER MARIO WORLD: SUNKEN GHOST SHIP
This level wasn't hard for people who are good at this game, but not for most of the people for example me. When I played this level for the first time, I always used to die in the section where ghosts appear randomly. I usually fall into the ghosts and die then. That's why I left my game progress inside that goddamn level.

3. SONIC UNLEASHED - EGG DRAGOON BOSS STAGE
During this stage, your skills of agility and timing is examinated here. You have to be fast and powerful, or else you'll be defeated.

Wario Land 4 - Several levels on Super Hard Mode, yeah I know its called "Super Hard" but still some of the things they do... These are the 3 worst imo
-Monsoon Jungle: Very tricky platforming that you have to do quickly due to jumping alligator enemies. Falling means you have to start the section again. Also you begin every level with ONE hit point, therefore you really cant afford to get hit at all. Also in a couple spots are some really unfair enemy placements.
-Hotel Horror: I hate those axe enemies. Way too many of those things.
-40 Below Fridge: This level is one of the hardest. All those damn icicles and those Damn Dirty... Yetis. Also the time limit is extremely short, especially if you try to get everything.
Also Wario Land Shake Dimension - Any level with a No Damage mission after World 3, which is pretty much all of them. Note: Im calling the mission difficult, not the actual levels.
I feel like level editor/romhack stuff doesn't really fit in here, you know? I mean, anybody can make levels in stuff like this as hard as they want - it's more impressive to me I guess when ultra-difficult levels are included in retail games.
And even that level doesn't come close to the insanity of Item Abuse 3 - a Super Mario World romhack so difficult that it's consider difficult just to complete on TAS.

I actually remember reading something stating that the game was created to be that difficult, making you rely on the Egyptian god cards. Either way, it's still a pretty unfair game when you still have to tribute 3 monsters to summon any of them. Thank god for Phoenix mode, though I actually have beaten reshef without phoenix mode a few times before.

I mentioned item abuse 3 earlier, but it's a hack and isn't technically part of an official game whereas Val's shellspace is actually in a Nintendo game (albeit the one least qualified for this thread)
Also, IA3 was meant to be TASed and the game of it is getting the right frame perfect inputs with a frame perfect bot, whereas with Mario Maker levels you're getting stuff you have to do legitimately, with your own hands.
