How do I change my name on the forum?

Wanted to change it to "Axotic Boo" to match my youtube channel (
http://goo.gl/FrFgTM).
I saw joeys post stating that he allows name changing, but I have no clue how to do it. Any help is appreciated, thanks!

You have to ask a staff member to change it for you.

Thanks! Ill be sure to do that next time I might want a name change (I probably never will wont another name change).
