Looks really cool, although I recommend using a slightly darker fog for the cave.
But that's just my unimportant opinion.

A spooky forest and cave themed level. The ideas in this level were nice and creative; however, there are some major flaws such as being forced to do a running jump just to make it to certain areas, too much fog, and the moving platform part.

Design: The design was ok, I can tell that you made an honest attempt to make the level as detailed as possible. I didn't see any cutoff or clash, and there was a great amount of variety of NPCs in the level. The only problems I have with this level are the jumps (some I had to do a crouch jump to make it across and others I had to do a running jump just to make it up to where I needed to go. The moving platform section in the goal was my biggest pet peeve in the level. If you kill the koopa troopa hovering over the 1st platform, you are officially screwed and you will have to restart the level since it is impossible to make the jump to the second platform without it. Also there was waaaay too much fog. I had a hard time seeing what type of special block I was hitting since the fog was blocking some of them. Also since there was so much, it was pretty disorienting.
Score: 5/10

NPC Placement: The NPC placement was pretty good, but there were some moments when I would advance and then suddenly BOOM!! this boo that chases you appears out of nowhere and you're either dead, or you take a hit. The coin placement and the item placement were nice, but I think about 1 or 2 more power-ups would have been nice to have in the level. I didn't see any NPC spam either, just a few random surprises here and there.
Score: 7/10

Music: The music was very fitting and the SMW cave music is pretty self explanatory. I'm pretty satisfied with the music choice, didn't see a problem there.

Atmosphere: The level was great with the atmosphere until the cave section. Since the level was called forgotten plains, about 40% of the actual level was the plain and honestly it looked more like a forest to me, but that's just me lol. I think swoopers would have increased the spooky atmosphere even more in the cave section. But again, just a suggestion
Score: 6/10

Overall score: 7/10 Pretty Good
This level does have potential, but there's plenty of room for improvement.

This is actually a pretty good level for the most part. The pits aren't too short (although the first one is a bit too long), the fog is frequent but not overdone, block placement is well done, and it's always clear where the NPCs are so the player doesn't take any unfair hits. In fact, the only reason this isn't a pretty good level is because of the moving platform section. Not only is the Koopa Paratroopa required to make it to the next line rider, like PROX wrote, but it's positioned in such a way to where it doesn't seem like it's necessary to continue. That's a pretty big design flaw, and I'm surprised you didn't catch it during testing. I killed it without thinking (by tossing a shell), then wondered why the heck I couldn't make the jump to the next platform. In the end, I had to abandon my Yoshi to progress. In conclusion, I'd say this deserves a 5.5/10.

Also, you forgot to sprite swap the Koopa's effects.
