Here we are.
How to create a black hole.
Way 1
1. Grab a planet.
2. Crush it into a tiny space.
3. Done.
For the earth, squish it down to 8.7 millimetres, or about the size of a housefly. For larger planets, increase the size. For smaller planets, decrease the size.
Way 2
1. Grab a piece of paper.
2. Make it black.
3. Cut it out.
4. Done.

There's an issue with your second method. By cutting a hole in black paper, the hole would be whatever colour the surface the paper is being held over is...

But couldn't you theoretically make the protons,neutrons, and electrons move closer to the nucleus and increase the atom's density?

Not enough though. They would likely collapse in on themselves first before you could even get close to that
