My Review:
Atmosphere [5/5]
This level has a lot of flaws, too many to accurately call out, but you did nail the atmosphere is perfect. The music really brought out the chilling theme this level was trying to pull off.
Level Design [1/5]
The level focused only on being difficult instead of being original and fun. Constantly having to climb vines in a low-ceiling area and fall over and over until you either quit or beat it eventually. There are way too many bullet bills in this level, let alone too many bullet bill blasters; a few of which you can actually get stuck between and never be able to die.
Also, this level is WAY too long. You need to shorten this a LOT. There is absolutely no need to have to climb for a minute and a half to enter a pipe, or climb blocks over blocks over blocks just to fall 27 blocks down. This wasn't fun, it was only tedious.
Graphical Continuity [1/5]
The tileset was nice, but you didn't use it even remotely close to how you could've used it. Sections just used a carbon-copy version of itself, as if it was just a repeating pattern. Add some visual distractions within this walls to help add some enemies in the walls or add some variety in the patterns in the walls. This was only one of the various issues I had with the level's graphical choices.
The second issue I had was the broken pipe. For one, the cracks would mean the pipe is unsafe despite every single pipe is like this, let alone they looked like they were made in MS Paint. I highly recommend simply removing this custom graphic and just use a default one. This message rings true on the bullet bills as well, these were simply ugly and didn't really fit with the level itself. I understand why there were there, but they didn't fit.
BGO Placement [0/5]
I have to give this a zero only because they had no impression on the level itself. You most prominent usage of BGOs weren't even BGOs but rather the vines, as they had a bigger impact of the level than anything else.
NPC Placement [0/5]
-Thwomps that only activate only when you commit to a jump and get hit by it.
-Bullet bills and erries placed randomly and without much reason behind it, on top of the incredibly long and dull level.
-Bullet Bill Blasters that you can stuck inside of and have to quit the game in order to restart, as they cant exactly kill themselves.
-Vines, which are NPCs, are placed in a nice fashion but you need to be absolutely perfect to actually jump across the vines.
-A p-switch that served absolutely no purpose.
Difficulty Replayability [1/5]
I can only give this a point because I really liked the atmosphere of the level. The difficulty is a mixture of way too easy and then just too tedious for no reason what-so-ever.
My hint, focus on making the level fun. This isn't Mario Maker, we don't make levels just to be difficult for the sake of it. Most of the time, people generally want to play a fun level not a long and tedious one.
