A bad item in a game? Well, games are meant to have challenges, so it's subjective, but let's consider a frustrating item:

Fangame: Downloading a fangame and discovering it's a Kaizo game can be really frustrating. This has happened to me with Mario hacks.

Super Mario Bros 2: In the final level, the mask that always helped you progress suddenly turns against you! That was a low blow from Nintendo & Fuji Television* to include that in the game. I remember when I reached that part, I had only one life left, did everything to avoid dying, and ended up dying in the most unfair way.

*Explanation: SMB2 is actually Yume Kōjō: Doki Doki Panic (Developed by Nintendo in cooperation with Fuji Television) . Nintendo made some changes to the game, replacing the original protagonists with Mario series characters and making a few alterations. For example, the magic potion was originally a genie lamp in Doki Doki Panic, serving the same purpose – it's just a graphical difference.

The stick in Dr. Jekyll and Mr. Hyde, it's as useless as an actual stick.

P-Balloon (Super Mario World Variant) lets just admit it. The P-balloon is just straight up the worst items in the whole mario franchise, probably the whole game industry, all that it does, is to give you a bit of control with a swinging pattern in a limited time, im glad SMM2
fixed this piece of garbage.

Mistake(Paper Mario 64)
this is the worst healing HP and FP in the game, only restores 1HP and 1FP. Thank god this wasn't featured again once Sticker Star And Color Splash came out.

doom 1 and 2's pistol is dreadful to use. it would be one thing if it were fine but outclassed by other weapons, but there is seriously nothing fun about using it. its awful damage output and slow firing speed would frankly be at least somewhat tolerable if it weren't for the horrendous accuracy. shots are 100% accurate if you wait for the firing animation to complete rather than holding the fire button, but it makes the already slow firing speed unbearable. also to add insult to injury doom's hitscan detection is wonky so sometimes you can just. miss even if you hit a shot dead-on. not a big problem for weapons with more pellets per shot or higher firing speeds but for the pistol every agonizingly weak shot counts
admittedly i like it from a design standpoint as someone who plays maps from a pistol start (which is starting maps with a pistol and 50 bullets rather than carrying over your previous weapons). the fact that you start with a functionally useless weapon gives mappers the ability to start you with any weapon they want, which adds a hell of a lot of variety to maps. however using it makes me pop a blood vessel. if you ever require me to use the default pistol on more than like three zombiemen you're on my shitlist
